To submit a rebate claim you will need:

  • A Complete Rebate account
  • Your insurance information
  • Information to verify your medication and cost*

Rebates are available only to:

  • Patients with commercial prescription coverage
  • Patients who are self-insured

Rebates are not available to:

  • Patients whose health insurance company prohibits participation in the program
  • Patients enrolled in Medicare Part D, Medicaid, VA, TriCare, CHAMPUS, or any other similar federal or state program
